We welcome donations by check. Please send your check and correspondence to our mailing address, along with the following information (we will not share your information with anyone without permission):

  • Your name
  • Your postal address
  • Your email address
  • The most appropriate way to address you (and if applicable, other contributors to your donation).  For example, Mr. Jon Smith or Mr. and Mrs. Jonathan Smith.
  • Indicate that this dedication is "on behalf of"
  • Name of dedicatee
  • Mailing address of dedicatee (if you would like them to receive an acknowledgement by mail).
  • Email address of dedicatee (if you would like them to receive an acknowledgement by email).
